Create or edit Debrief form templates

This guide covers configuring form fields, managing template visibility, and customising debrief forms for your team.

Managing debrief forms in Performance Leader gives you the flexibility to tailor feedback processes to your firm's needs. Whether you're creating new templates from scratch or updating existing ones, this guide will walk you through each step of the configuration process.

Accessing debrief form configuration

Before you begin creating or editing debrief forms, you'll need to access the configuration area where all template management happens.

  1. As a Super Admin, navigate to Manage Apps in your admin menu

  2. Click on Debriefs

  3. Select Configuration and Tools

  4. You'll see a list of any existing debrief forms

Note: You can change the order of forms by clicking on the six-dot menu and dragging them into your preferred sequence.

Editing an existing debrief form

Once you've accessed your debrief forms, you can modify any aspect of an existing template to better suit your requirements.

  1. Click the Edit Type option for the form you want to modify

  2. A menu will appear where you can update:

    • Form name

    • Description

    • Visibility settings (determines if feedback respondents can see each others' feedback when submitting theirs)

    • Archive status (enable or disable the form)

  3. Scroll down to see the different input sections:

    • Inputs required when the form is set up

    • Inputs required for team members to provide feedback

    • Inputs for the team leader to provide final feedback

  4. Click Update Type to save your changes

Modifying debrief form inputs

Each section of your debrief form can contain multiple inputs that you can customise to capture the information you need.

  1. Within the form editing screen, locate the input you want to modify

  2. Click the Edit option for that input

  3. You can adjust:

    • Input type (text box, dropdown, date picker, etc.)

    • Title of the field

    • Instructions to guide users

    • Whether the field is required or optional

  4. Use the toggle options to reorder inputs within the form

  5. Click Update Type when you're satisfied with your changes

Creating a new debrief form

Building a new template from scratch allows you to design a debrief process that perfectly matches your team's needs.

  1. From the Configuration and Tools screen, click Add type

  2. An empty dialogue will appear for you to configure your new form

  3. Provide all required fields:

    • Form name

    • Description

    • Visibility settings

    • Input sections for setup, team member feedback, and team leader feedback

  4. Configure each input field with appropriate types, titles, and instructions

  5. Click Create type to save your new form

Once created, your new debrief form will be available for users when they're setting up debriefs.
