How to use Phamily to Bill for APCM

Follow these guidelines to download the Phamily billing report and submit claims for APCM services.

What are the requirements to bill a patient for APCM?

The following must be met in order to bill for APCM services:

  1. Your Practice meets the 10 requirements to provide APCM: Toolkit - APCM Checklist.pdf

  2. Patients must have a Care Plan that has been shared with them

  3. Patients must have provided consent to participate in APCM

When should I submit claims for APCM?

You can bill for APCM services once per patient per calendar month. Claims can be submitted at any time during the month the claim will be associated with, or during subsequent months. Note, commercial insurers may require claims to be submitted within 90 days from date of service. It’s recommended to verify specific policies with each payer to ensure compliance with their submission deadlines.

How do I download the Phamily Billing Report?

1. Click on Enrolled

2. Use the Calendar at the top of the page to select the month you want to download the billing report for

3. After you’ve selected a month, click on READY TO BILL next to APCM

4. Click on Download at the top right of the page

5. Click on Download Billing

6. On the pop up that appears, Click on DOWNLOAD to save the file to your computer
