Most patients will receive the consent link during outreach. In the event you are drafting an outreach or manually attempting to collect consent, follow the instruction below.
How To send a Consent URL to a Patient
Go to the Patient Profile
Enter {{apcm_consent_url}} in the body of the message you're sending. Once sent, patient will see an unique consent url created on their end.

Each Patient has a unique Consent URL. Sharing a URL between Patients WILL NOT work.
What you enter: {{apcm_consent_url}}
What patient sees: https://my.phamily.com/my/consent/`UniqueURL`
NOTE: Verbal consent is acceptable for APCM. Make sure that verbal consent is properly documented in Phamily.
Documenting Verbal consent
To document verbal consent, do the following:
1.Navigate to the Patient profile > Click on the clipboard icon
2. Click on Document Additional Consents
3. Click the + icon
4. Click on Verbal - Consent given over phone call
5. Click on Confirm
