Gathering APCM Consent

Learn to use Phamily's APCM consent flow, and save administrative time on consent processing, authenticating and recording.

Gathering APCM consent is only necessary for Practices using Phamily for APCM. To learn more about APCM, reach out to support@phamily.com to be connected with our Team.

Most patients will receive the consent link during outreach. In the event you are drafting an outreach or manually attempting to collect consent, follow the instruction below.

How To send a Consent URL to a Patient

  1. Go to the Patient Profile

  2. Enter {{apcm_consent_url}} in the body of the message you're sending. Once sent, patient will see an unique consent url created on their end.

🚨🚨 DO NOT COPY & PASTE UNIQUE LINKS

Each Patient has a unique Consent URL. Sharing a URL between Patients WILL NOT work.

What you enter: {{apcm_consent_url}}

NOTE: Verbal consent is acceptable for APCM. Make sure that verbal consent is properly documented in Phamily.

Documenting Verbal consent

To document verbal consent, do the following:

1.Navigate to the Patient profile > Click on the clipboard icon

Click on the clipboard icon

2. Click on Document Additional Consents

Click on Document Additional Consents

3. Click the + icon

Click the + icon

4. Click on Verbal - Consent given over phone call

Click on Verbal - Consent given over phone call

5. Click on Confirm

Click on Confirm
