Completing Enrollment & The Patient Profile

Complete your patient’s profile and enroll them in CCM.

Follow the steps below to complete your patient’s profile and enroll them in CCM. For best results we recommend completing these steps at the same time.

Enroll in CCM

1. Navigate to the patient's profile and click the ENROLL button.

2. The pop-up window that appears will be pre-populated with a standard CCM welcome message. Click ENROLL & SEND MESSAGE to send the text.

If the patient does not have a cell phone or prefers not to text, you can still enroll them in CCM! Just click the dropdown menu to the right of the enrollment link and select "Enroll Without Welcome Text".

Update PHI Authorization status

You can collect the patient's PHI Authorization with either a written consent, or by having them complete Phamily's Connected Consent form. Once consent is collected, follow the instructions below to update Phamily.

1. Locate the patient's profile in Phamily and click the gray No PHI Auth button.

2. On the pop-up window that appears, click CONFIRM to indicate that the patient has given PHI authorization.

Complete the Profile in Phamily

To effectively manage the patient in Phamily, it's important that you complete their profile after enrolling. Key profile fields and their uses are below.

Patient demographics: Name, date of birth, and phone number. The green phone icon indicates that the patient's phone number is SMS capable.

Additional remarks: Use for free text notes or reminders to the care team. Great for making note of the patient's caregiver, their communication preferences, or outstanding items.

Tags: Click the tag icon to add custom tags to the patient’s profile. Tags are best used for categorical entries that you would like to use for patient management (i.e. Needs AWV, Has Caregiver, Diet Management, Exercise Management, Losing Weight, etc.) You can use tags to filter large patient lists or send custom check-ins. To remove a tag, simply click the blue tag icon to the right of the Patient's name and deselect the tag you want to remove. If you would like to have a tag completely removed from the Phamily account, send the name of the tag(s) to support@phamily.com.

Provider and care manager: Update these fields to reflect the patient’s care team.

Medical summary: Use these fields to quickly view the patient's chronic conditions and medications while in Phamily. Details and best practices are in the next section.

Complete the Medical Summary

Click the two arrows on the left side of the Medical Summary category to expand the section.

Here are best practices for populating the patient's medical summary:

Conditions: Populate the patient's two chronic conditions, then any other conditions of focus. This section is important because conditions drive Phamily's AI-driven check-in content. The more conditions populated here, the more varied the messages Phamily sends to the patient!

Diagnoses: Add the two or more ICD-10 codes corresponding with the patient's chronic conditions. These diagnosis codes are populated on the billing report each month.

Active Medications: Ensures any medications of importance or quickly visible without having to enter the patient's chart and enables care plan completeness.

Allergies: Ensures that any allergies of note are quickly accessible by the care management team.

Patient Profile FAQs

How do I add or edit a Patient's Insurance

To make changes to a Patient's insurance, navigate to the Patient Profile and click the settings gear to the right of the Patient Name > Show more fields. Click into the insurance field. The insurance field is free text, so you can add any insurance that isn't present. Note that this field is only for insurance names, not Policy Numbers or IDs.

How many conditions/diagnosis should I add to the Patient profile? Do I add every condition listed in the EMR?

  1. Conditions: In this section, we'll focus on 2-5 specific conditions that we'll be supporting in the patient's management plan. These will help our auto-suggested check-in messages.

  2. Diagnosis: Here, we'll list at least the two chronic conditions that render the patient eligible for our program, as indicated in your Electronic Health Record.
