Sending Check-ins

Use Check-ins to reach hundreds of patients at a time.

Phamily's check-in feature allows you to proactively engage many patients at once. To maximize patient engagement and program success, we recommend sending two check-in messages a week.

Sending a Check-in Message

To send a Check-in Message, do the following:

1. Click on Check-ins

2. On the Check-ins page, select the Patients you want to send a Check-in message to.

By default, Phamily will send a check-in to all eligible patients, clicking the highlighted areas allow you to select another set of Patients

On the Check-ins page, select the Patients you want to send a Check-in message to.

To send check-in messages to all patients assigned to a specific care manager when engaging multiple providers’ patients, hover your mouse over their name and click the lock icon that appears.

EXAMPLE: Care Manager Carla wants to send a check-in to all patients assigned to her, regardless of the provider.

By clicking the Practice name at the top of the Check-ins page, you can select a specific Provider or Care Manager's Patients to include in your check-in.

3. Select the type of Check-in you want to send.

Before you get started, decide which check-in message to send:

  • Auto-Suggest: Phamily recommends the best Check-ins for each patient based on their conditions (learn how to populate conditions here).

  • Custom: Use Custom messages to send themed messages to patients. For example, the Lifestyle module will engage patients about leading a healthy lifestyle, diet, and exercise.

Select the type of Check-in you want to send.

Note: For best results, ensure that Patient's have a complete Medical Summary.

4. Click on Review at the top of the Check-ins page

5. Click on each suggested check-in to review the check-in content and recipients

6. Review the message preview.

7. To view message recipients, click Selected Patients.

Clicking the X to the right of the Patients name will remove them from the Check-in.

The "Review Check-ins" page allows you to view drafts of the messages as well as preview the patients that will be receiving them. You can make the following changes before sending check-in messages.

Change message content: Click the check-in name at the top of the message to preview and customize before sending.

Change message recipients: Click the Selected Patients icon to view a list of patients that will be receiving the message.

  • Remove individual recipients by clicking the X next to their name. You can remove groups of recipients by clicking the X next to any category at the top of the patient list.

  • Add recipients by dragging any tag from the right side of the page to the message. All patients marked with that condition/tag will be included in the Check-in.

8. Once you're satisfied with the Check-in, Click Send

Things to Note:

  1. Please refrain from sending any links via the mass check-in message.
    Sending links will result in a higher deliverability error rate.

  2. Patients can only receive one check-in message every 18 hours

Tips for making your check-ins count

  1. Phamily recommends sending two check-ins a week: one custom message and one auto-suggested/module. We will regularly create and share custom message templates for you to use.

  3. Always include a question at the end of your messages to keep the conversation going.

  4. Changed your mind about the check-in you wish to send? Click Start Over at the top of the page to go back.

  5. To check the performance of your check-in, check the Inbox Summary
