How to Send Custom Check-in Messages

Give your weekly check-ins a personal touch. Use Phamily to send custom messages to hundreds of patients at once.

To maximize engagement and keep the conversation going, we recommend sending your patients two check-in messages a week. Phamily allows you to send automatic check-in messages driven by your patients' conditions, but occasionally your practice may need to send a custom message to a group of patients.

You can use Phamily's custom check-ins to send office updates, Happy Birthday messages, notes about month-specific topics (American Heart Month, Lung Cancer Awareness Month, etc.), and more!

Sending A Custom Check-in

To send a custom check-in do the following:

1. Click on Check-ins

2. On the Check-ins page, select the Patients you want to send a Check-in message to.

By default, Phamily will send a check-in to all eligible patients, clicking the Practice name and Program name at the top of the page allow you to select another set of Patients

On the Check-ins page, select the Patients you want to send a Check-in message to.

By clicking the Practice name at the top of the Check-ins page, you can select a specific Provider or Care Manager's Patients to include in your check-in.

3. Click on Send Custom Check-in

If you want to send a custom check-in using a template in your account, you can click Skip to go to the next screen.

4. Select your desired Check-in template. You can search for a template using the search bar in the top left

5. Drag your Patients to the check-in Template. On the right side of the screen, you will see Patients grouped by condition, tag, language, demographics, and the number of CCM minutes they have logged for the month. You can also search for Patients individually using the search bar on the panel.

Additionally, you can write your own message without using a template. Simply click into the message field and compose your message.

6. Once you are satisfied with your message and Patient list, click Review

7. Click on Review Next Check-in

8. Click on Send

Things to Note:

  1. Please refrain from sending any links via the mass check-in message.
    Sending links will result in a higher deliverability error rate.

  2. Patients can only receive one check-in message every 18 hours

  3. To monitor the performance of your Check-in, check out the Inbox Summary
