Focus Filters are designed to help you quickly find and prioritize the right patients to engage — whether you're just starting your day or wrapping up your month. With just a few clicks, you can see which patients need your attention based on engagement activity and time thresholds.
You’ll find Focus Filters in the Enrolled tab. From left to right, they’re organized to support your workflow across the month:
Follow Up
See patients you've started engaging but haven’t completed care milestones with yet.
Review
Find patients with care needs that still need to be addressed before the month ends.
15+ / 35+ / 55+ Minutes
Quickly check in on patients reaching monthly time thresholds.
Re-engage
Highlight patients who haven’t replied and may need a follow-up call or personalized nudge.
These buttons are dynamic and change depending on the time of the month.
Early in the month: Start with filters on the left (e.g., Follow Up, 15+ Minutes) to build momentum.
End of the month: Shift to the right (e.g., 35+ and Review) to close out remaining care milestones.
To view newly enrolled Patients, click the button labeled New. Clicking the arrow next to New allows you to filter for various enrollment statuses:
Select "Unenrolled" to view patients who were unenrolled during the current month.
Filter for Patients with consent documented as well as consent not documented.
To view Patients without Care Plans, click the No Care Plan filter. To further refine results, you can use the Last review date column to identify when each patient’s care plan was last updated. This can help you prioritize updating overdue care plans or tracking recently reviewed ones.
Care plans are required for billing, so it's important to understand which patients have them and which patients need them.
Patients with no care plan includes all CCM-enrolled patients without a care plan in Phamily.
Patients with 20+ mins and no care plan shows all patients with 20+ minutes of care management time logged. You should prioritize creating and sharing care plans for these patients by the end of the month.
To view Patients with Care Plans that have not been shared, click on Not Shared
To view Patients by their PHI Authorization status, click the No PHI filter
Patients must have a signed PHI Authorization form to be sent Protected Health Information via text message. Use Phamily's No PHI filter to quickly view all patients who have not given signed PHI consent.
To view Patients by their Engagement status, use the Patient Engagement filters in the top right. This filters for engagement via text or phone call.
Note: You must check off Spoke to patient on the phone when creating an activity for the phone call engagement date to update.
To view Patients without a Care Manager, click the "Without Care Manager Button at the top of the Enrolled page.
To view Patients by Insurance, filter by the insurance column. Multiple insurances can be selected at once.
Note: The list of insurances will be generated based on the insurance plans that your patients have.
Additional Filters
Phamily has additional filters that can be used to view your Patient Panel. Additional filters include:
Language: Filter for Patients who speak a certain language
Conditions: Filter for Patients with certain conditions
Program: Filter for Patients in a certain Program
Est. Mins: Filter for Patients within a specified range of Care time provided
Last Contact: Filter for Patients based on Last Contact date. Note that in the context of the Last Contact filter, Response pertains to the last text message received, while Engaged pertains to a Phone call.
Exporting Filtered Results
Once you've applied filters, Phamily allows you to export the filtered records. Simply click the Download button on the Enrolled page and download the data for external review. The export will indicate whether care plans exist (e.g., marked 'Yes'), but specific review dates will only be visible in Phamily’s interface.
