When reviewing a patient chart, always look to see if the patient has already consented. You can see the status at the top of the chart near their DOB

In order to update the consent status, just click on the badge and confirm in the pop up window in order to confirm the change in status.

Finding Consent Documentation

There are a few different places to find documentation for consent depending on your practice's workflow:

EHR

If your practice obtains consent through your EHR patient intake process, please check your EHR and update in Phamily accordingly.

Dropbox

If your practice has done consent in collaboration with Phamily through text outreaches, please review your Dropbox folder for the patient's file and update in Phamily accordingly.

In Office

If you have an in person process for obtaining consent, please review those files and then update in Phamily accordingly.

Obtaining Patient Consent

If you have already sent a link to the patient, you can send a message and remind them to sign the form. If they still haven't responded, we recommend calling them, especially if they have already engaged via text.

NOTE: The Public Health Emergency is ending on May 11, 2023. Once it ends, every patient must have authorized PHI in order to be texted via Phamily.
