Configure FRAT templates
This article covers how to build Flight Risk Assessment Tool (FRAT) forms, set risk thresholds, and manage mitigations in Safety Hub. It is written for admins who hold the Configure FRAT permission (canConfigureSafetyFRAT).
Before you start
To configure FRAT templates, your FSP role must include the Configure FRAT permission. Assign it in FSP under Settings → Users → Roles & Permissions → Edit Role → Safety Hub section.
Open FRAT Settings
In the Safety Hub left navigation, go to Settings → FRAT.
The FRAT Settings page has three tabs: Risk Thresholds, Forms, and Mitigations.
Set risk thresholds
Risk thresholds define the score ranges that map to named risk levels (for example, Low 0–20 / Medium 21–50 / High 51+). Safety Hub compares a pilot's total FRAT score against the assigned threshold to determine the risk level shown on the assessment.
The Risk Thresholds tab lists your threshold configurations. Each configuration shows its named bands as colored chips with their score ranges and a count of how many forms use it. You can add custom bands, rename levels, and pick a color for each level, so the labels and palette match your operation's language. Per-form assignments are set inside each form's builder (see Build a FRAT form below).
Create a threshold configuration
On the Risk Thresholds tab, select New Configuration.
Name the configuration (for example,
FAA VFR,Night Ops,IFR).Define the named levels. For each level: - Enter the score range. Ranges must be contiguous and cover all possible scores. - Rename the level and pick a color if you want it to differ from the defaults. - Add custom bands if three levels aren't enough for your matrix.
Save the configuration.
Tip: Create separate threshold configurations for flight-rules categories that carry different risk tolerance. Assign each FRAT form to the configuration that matches its intended use.
Build a FRAT form
FRAT forms are the question sets pilots answer before a flight. Each option in a form carries a numeric risk value; Safety Hub sums these values into the total score.
Forms are versioned. Publishing a form does not disturb in-progress assessments on the previous version. The Forms tab summarizes how many forms are live, draft, and archived.
Create a new form
Select the Forms tab.
Select Create New Form. A template picker opens with a Blank option plus built-in templates. Choose one to start from. (Duplicating an existing form is a separate action — see Duplicate a form below.)
Enter a form name (for example,
Day VFR,Night VFR,IFR).Add questions. For each question: - Write the question text. - Add answer options. - Assign a risk value to each option. Higher values indicate higher risk.
Assign the form to a Risk Threshold configuration. This per-form assignment is what maps the form's total score to named risk levels.
Select Publish to make the form available for new assessments.
The Forms tab lists live forms and archived versions. Archived forms remain readable on historical assessments but cannot be selected for new ones.
Duplicate a form
Duplicating is the fastest way to create variants (for example, a Night VFR version of your Day VFR form):
On the Forms tab, find the card for the form you want to copy.
Open the card's overflow menu (the kebab / ⋮ icon) and select Duplicate. You do not need to open the form first.
Rename the copy and adjust questions or risk values as needed.
Publish the new form.
Manage mitigations
Mitigations are options a pilot can apply during an assessment to reduce the calculated risk score. Each mitigation carries a Risk Reduction value (shown as -N pts) that offsets the initial score. The final (post-mitigation) score determines the risk level shown.
Safety Hub displays the Risk Summary on each assessment as three values: Initial / Mitigation / Final.
The Mitigations tab groups mitigations by category and shows summary tiles for Total Mitigations, Active, and Categories. Each mitigation shows its Risk Reduction value (as -N pts) and an active/inactive toggle — only active mitigations appear to pilots during an assessment.
Add or edit a mitigation
Select the Mitigations tab.
Select Add Mitigation (or select an existing mitigation to edit it).
Enter: - Name — shown to the pilot during the assessment. - Description — optional detail about the mitigation. - Category — groups related mitigations in the picker. - Risk Reduction — the number of risk points this mitigation removes, shown as
-N pts. - Active — use this toggle to activate or deactivate a mitigation without deleting it.Save.
⚠️ Deleting a mitigation removes it from future assessments. To take a mitigation out of rotation without losing it, switch it to inactive instead.
Where pilots complete assessments
Pilots can complete FRAT assessments from a reservation's Preflight suite, where completing a form is tied to being assigned to the reservation near the scheduled event. This reservation-linked behavior is handled outside of Safety Hub's web settings, so there is no edit-window setting to configure here.
The Risk Assessments console (Safety Hub → Risk Assessments) is always accessible to users with Access Safety Hub and shows all assessments as a management and record layer. See the "Complete a FRAT assessment" article for the pilot-facing flow.
Activate and deactivate forms

Only published forms appear in the Add Assessment modal's form picker.
Recommended setup sequence
Follow this order when configuring FRAT for the first time:
Risk Thresholds — define your score ranges first, so forms have a threshold to assign.
Mitigations — build your mitigation library before building forms, so you can confirm Risk Reduction values will meaningfully reduce scores.
Forms — build and publish forms, assigning each to its threshold.
Test — create a test assessment using each published form and confirm the risk level, mitigation math, and AI Conditions Summary behave as expected.

Troubleshooting
The FRAT item does not appear on a reservation's Preflight tab. Completing a FRAT from a reservation is assignment- and timing-gated near the scheduled event. Confirm the user is assigned to the reservation. If the reservation's Preflight tab does not offer an editable FRAT item, use the Risk Assessments console to view the assessment record.
"Enter a departure airport to generate a Conditions Summary" appears on an assessment. The AI Conditions Summary requires a departure airport. The pilot must enter a departure before Safety Hub can fetch METAR, AIRMET, and SIGMET data. This message clears once a departure is set.
A mitigation option does not appear during an assessment. Confirm the mitigation is active. Open Settings → FRAT → Mitigations and check its toggle. A deactivated or deleted mitigation is not offered to pilots.
A form is not available in the Add Assessment picker. Only published forms appear. Open Settings → FRAT → Forms, locate the form, and confirm its status is Live (not Draft or Archived).
