File a Safety Report
Safety Hub lets anyone submit a Safety Report — either from inside the app or through a public link your organization shares. This article is for pilots, crew members, and anyone else who needs to file a report or check its status.
What is a Safety Report
A Safety Report documents a safety event, near-miss, or concern. When you submit one, it routes automatically to your location's safety team for review. The safety team then assigns it, investigates if needed, and may link it to a Hazard or Corrective Action.
Every report captures a small set of core fields — such as Report Title, Reporter, Event Date/Time, Event Location, Event Narrative, etc. Your organization can also add optional standard fields — such as Report Type, Severity, injury information, aircraft damage details, and a Regulatory Reporting Status, amongst many others — and may add custom fields of its own.
Submit a report when you're logged in
Sign in to Safety Hub.
In the left sidebar, select Safety Reports under Safety Management.
Select New Report in the toolbar of the Reports page.
Fill in the reporter section of the form. Required fields are marked. Common fields include: - Report Type (e.g., Incident, Accident, Occurrence, Suggestion) - Event Date and Location - Severity (Catastrophic / Hazardous / Major / Minor / Negligible) - Injuries and Aircraft Damage - Regulatory Reporting Status — use this to flag NTSB Part 830 or FAA notification requirements - Description of what happened
Review your entries on the Review step to confirm everything is correct.
Select Submit Report when finished.
Your report is assigned a Report ID (formatted RPT-YYYYMMDD-XXXXXX). Save this ID — you can use it to check your report's status later.
Submit a report anonymously (public link)
Your organization can share a public Safety Report link that doesn't require a login. Anyone — including anonymous reporters — can use it.
Open the public link your organization provided. It opens a standalone submission form.
Fill in the reporter fields. Your email address is optional; you may enter
N/A.Review your entries on the Review step to confirm everything is correct. The form is protected by invisible reCAPTCHA, which runs automatically when you submit — there is no challenge to complete.
Select Submit Report.
The page displays your Report ID after submission. If you provided a valid email address (and did not submit anonymously), Safety Hub also sends you a confirmation email that includes your Report ID and a link to check your report's status. Either way, copy or write down your Report ID — you'll need it to look up your report's status.
⚠️ Save your Report ID. If you submit anonymously or without an email address, no confirmation email is sent, and your Report ID is the only way to check your report's status later.
Check your report's status
You can look up the status of any report using its Report ID — no login required.
Open Safety Hub (or the public link your organization shares).
In the Reports toolbar, select Check Report Status.
Enter your Report ID and submit.
The status lookup shows where your report is in the review process. Typical statuses progress from Submitted → In Progress → Under Review → Closed. Your organization may use different status names.
Fields on a Safety Report form
Every Safety Report guarantees seven core fields. Your organization builds the rest of the form, so the exact fields you see can vary.
Core fields (always present):
NOTE: these field labels can be renamed per your organizations desire, but will always be present at a minimum.

Fields in the Reporter Submitted Data section are locked after submission. The Safety Team works its own fields during review.
What happens after you submit
The report routes to the safety team for your location.
A safety team member reviews it, may ask for follow-up, and updates the status.
If the report reveals a broader hazard, the team may link it to a Hazard record and open a Corrective Action.

Troubleshooting
I can't find the New Report button. You need the Access Safety Hub permission in FSP to sign in, and your role may not include the ability to create reports directly. Contact your administrator. Anyone can submit via the public link without a login.
I lost my Report ID after a public submission. There is no way to retrieve a Report ID after you leave the submission confirmation page. Contact your organization's safety team directly and describe the event date, location, and what happened so they can locate the record.
