Manage Corrective Actions

Corrective Actions (CAs) let you record, assign, and track the steps your team takes to address a hazard. This guide is for safety officers, safety managers, and team members who create and work CAs in Safety Hub.

What a Corrective Action is

A Corrective Action is a task linked to a hazard. It captures:

  • What needs to be done (Action Type and Category)

  • Who is responsible (Assigned To)

  • When it's due (Due Date)

  • How success is verified (Verification Method and Effectiveness Rating)

Every CA requires a linked hazard. You can also link documents and attach supporting files. For background on how hazards are created and rated, see the "Manage Hazards" article.

Open the Corrective Actions register

In Safety Hub's left sidebar, go to SAFETY MANAGEMENT → Corrective Actions.

A row of stat tiles sits at the top of the register: Active, Overdue / Due Today, Awaiting Verification, and Completed. Below the tiles is a searchable, filterable list.

You can filter by Status, Priority, Operational Area, and Team. The columns show Report ID, Action, Hazard, Operational Area, Team, Priority, Status, Assigned To, Due Date, and Last Modified (the date and time the CA was last edited).

Priority values are Critical, High, Medium, and Low. Use Critical for the highest-urgency actions.

If the list shows "No corrective actions found matching your filters", clear your filters and try again.

Create a Corrective Action

You need the Manage Corrective Actions permission in FSP to create CAs.

  1. In the Corrective Actions register, select Add Corrective Action.

  2. Enter a title and description for the action.

  3. Set the Action Type: Corrective, Preventive, or Improvement.

  4. Set the Action Category: Training, Procedure Update, Equipment, Policy Change, Maintenance, Personnel Action, or Other.

  5. Set the Priority: Critical, High, Medium, or Low.

  6. Link the Hazard — this field is required. Select the hazard this CA addresses.

  7. Set a Due Date and assign it to a team member using the Assigned To picker.

  8. Optionally link relevant documents and upload attachments.

  9. Save the CA. It opens in Open status by default.

Tip: The assignee must be a Safety Team member in FSP to appear in the picker. If someone is missing, ask your admin to flag them as a Safety Officer in FSP.

Track and update a CA

Open any CA row to see its detail view. The Team, Assignee, and Status controls sit across the top of the page. The detail shows:

  • Details — Action Type, Category, description, key dates, estimated effort, and resources required.

  • Completion — the completion summary, actual effort, and the planned verification method.

  • Timeline — Action Created, Completed, Verified, and Action Closed events, plus time-to-completion metrics.

  • Linked Items — the linked Hazard (required) and linked Documents.

  • Attachments — supporting files, in its own sidebar section.

Change the status

Move the CA through your organization's configured statuses using the Status control at the top of the detail. Default statuses are:

Open

Newly created; work not yet started

In Progress

Work underway

Completed

Work done; awaiting verification

Verified

Verifier confirmed completion

Closed

Terminal — fully resolved

If a CA is past its Due Date, it shows N days overdue in the register.

Verify and close a CA

Verification confirms the action was completed and had the intended effect.

  1. Open the CA detail.

  2. In the Completion section, record the completion summary and the planned Verification Method.

  3. In the Verification section, enter the Effectiveness Rating.

  4. Move the status to Verified once the verifier has confirmed completion.

  5. When the loop is fully closed, move the status to Closed.

Closing a CA does not automatically change the linked hazard's risk rating. Residual risk is set manually on the hazard. For detail on how hazard risk ratings update, see the "Manage Hazards" article.

Track CA analytics

Select SAFETY MANAGEMENT → Analytics → Open Loop Items to see hazards that have CAs without verification — a key audit-readiness check. The Performance Assessment dashboard also surfaces CA completion rates for the Accountable Executive review.
