Manage hazards in Safety Hub
The Hazards register lets you identify, rate, and track the risks your operation faces. This article is for safety personnel who create and manage Hazard records, link them to safety reports, and connect corrective actions to close out risks.
What a hazard record contains
Each hazard record holds three main pieces of information:
Hazard Triangle — Source, Mechanism, and Target.
Risk Assessment — a 5×5 Likelihood × Severity matrix. You set an Initial Risk rating when you open the hazard, then a Residual Risk rating after corrective actions reduce the exposure. The record shows the percentage reduction after mitigations.
Linked Items — connections to the Safety Reports that surfaced the hazard and the Corrective Actions assigned to address it.
The record also shows a Timeline on the right, tracking key events such as when the hazard was identified, when residual risk was assessed, and when it was closed.
Your organization's admin configures the hazard types (Organizational, Environmental, Human, Technical, or custom) on the Hazards tab of Settings → SMS. Operational Areas are configured separately, on the General tab of the same SMS Settings page.
Open the Hazards register
In the Safety Hub left sidebar, select Hazards under the Safety Management group. The page is titled Hazard Management.
A row of KPI stat tiles sits at the top of the page: Open Hazards, High Risk, Closed, and Open Actions. Below the tiles is a searchable, filterable list. Columns include Report ID, Hazard, Status, Initial Risk, Residual Risk, Reports, Actions, Identified (the date the hazard was raised), and Last Modified.
You need the Manage Hazards permission in FSP to create or edit hazard records. Without it, you can view records routed to your team but cannot make changes.
Create a hazard
On the Hazard Management page, select Add Hazard.
Fill in the Hazard Triangle: - Source — the condition or object that gives rise to the hazard. - Mechanism — how the source leads to a harmful outcome. - Target — the person, aircraft, or operation at risk.
Choose a Hazard Type and Operational Area from your organization's configured lists.
Set the Identification Method — Reactive, Proactive, or Predictive.
Set the Initial Risk using the two dropdowns: choose a Likelihood (1–5) and a Severity (A–E).
Select Save to open the hazard record.
The hazard enters your organization's configured initial status (for example, Open) and is visible to members of your safety team and to your Safety Manager and Accountable Executive.
Link a safety report to a hazard
Hazards and safety reports have a many-to-many relationship. You can link from either side.
From the hazard detail: 1. Open the hazard record. 2. In the Linked Items panel, find the Safety Reports section and select Link Existing. 3. Search for the report by ID or title and confirm.
From a safety report: open the report, scroll to the Hazards section of its Linked Items panel, and select Link Existing (or Create Hazard to make a new one). See the "Manage safety reports" article for the full report workflow.
Linked reports appear under Safety Reports in the Linked Items panel. If no reports are linked yet, the section shows No safety reports linked.
Add a corrective action to a hazard
Corrective actions track what your team does to reduce or eliminate the hazard.
Open the hazard record.
In the Linked Items panel, find the Corrective Actions section and select Add Action to create a new one, or Link Existing to attach an existing corrective action.
Fill in the action details — type, category, assignee, and due date.
Select Save.
The corrective action appears on both the hazard record and the Corrective Actions register. See the "Manage corrective actions" article for the full corrective action workflow, including verification and effectiveness rating.
Update risk ratings
After corrective actions are underway or verified, update the Residual Risk rating to reflect the reduced exposure.
Open the hazard record.
In the Risk Assessment section, click a cell in the Residual Risk grid.
Select Save.
The record recalculates and displays the percentage reduction between the initial and residual ratings.
Move a hazard through its status workflow
Status labels are configured by your admin. Typical statuses are Open → Under Review → Mitigating → Risk Accepted → Closed. To advance the status:
Open the hazard record.
Select the current status label and choose the next status from the dropdown.
Closed is a terminal status. A closed hazard is locked for editing — change its status to re-open it before making edits. The hazard remains in the register and in linked report and corrective action records for audit purposes.
Review hazard analytics
Select Analytics → Open Loop Items in the left sidebar to see which hazards have no linked corrective action. This view flags gaps in the closed-loop safety process.
The Performance Assessment and Repeat Events dashboards also surface hazard-related patterns across your operation.
Troubleshooting
A person doesn't appear in the assignee picker. They need to be flagged as a Safety Officer on their FSP user record. Ask your FSP admin to set that flag. Safety Hub refreshes the picker list from FSP each time it opens.
You can see a hazard but can't edit it. Either you lack the Manage Hazards permission, or the hazard is routed to a team you are not a member of. Contact your Safety Manager to be added to the relevant team or to have your permissions updated.
The hazard shows no linked reports. No safety report has been linked yet. Use the Safety Reports section of the Linked Items panel to add one, or link the hazard from the report detail.
