Safety Hub for Accountable Executives
Safety Hub gives the Accountable Executive (AE) a read-only view across every safety record in the organization — reports, hazards, corrective actions, and analytics — without requiring any editing permissions. This article is for the person designated as the AE under FAA Part 5.
What the Accountable Executive role means in Safety Hub
FAA Part 5 requires exactly one Accountable Executive per organization. In Safety Hub, the AE designation does two things:
Visibility bypass. You see every safety record across all Safety Teams, regardless of which team owns it. No one has to route a record to you for you to read it.
CA assignability. Corrective actions from any team can be assigned to you directly.
Your day-to-day access is read-only. You do not edit reports, hazards, or corrective actions — that work belongs to Safety Managers and Safety Officers. Your role is oversight and accountability.
Prerequisite: An FSP administrator must designate you as Accountable Executive under Settings → Safety Team in Safety Hub, and your FSP user record must be flagged as a Safety Officer. If you cannot access Safety Hub, contact your administrator.
Access Safety Hub
After signing in:
Open Safety Hub.
The Safety Dashboard loads automatically.
The dashboard opens with a Needs Attention callout row of clickable pills (overdue reports, high-risk hazards, overdue actions, overdue document reviews), a row of KPI stat tiles (Total Reports, Open Reports, High Risk Hazards, Overdue / Due Today Actions, Overdue / Due Today Reports, Regulatory Pending, Unassigned, Oldest Open, Document Reviews), and interactive charts (Reports by Status, Severity Distribution, Report Trend, Reports by Operational Area). Use the time-range selector — Today / 7D / 30D / 90D / 1Y — to change the period.
Review safety records
As AE you can open any record to read it. You cannot change status, assign members, or edit fields.
Safety Reports
Go to Safety Management → Safety Reports. The queue shows every report across all teams, with columns for Report ID, Title, Reporter, Event Date, Status, Team, Assigned To, Due Date, and Last Modified. A report moves through the default flow Submitted → In Progress → Under Review → Closed, where Closed is the terminal status.
Select any report to read the full detail: the reporter's submitted data, the safety manager's fields, comments, timeline, linked hazards, and audit history.
Hazards
Go to Safety Management → Hazards. The page is titled Hazard Management and lists initial and residual risk ratings, linked reports, and corrective actions for each hazard.
Select a hazard to see the Hazard Triangle (Source / Mechanism / Target), the 5×5 risk matrix, and the full corrective action list.
Corrective Actions
Go to Safety Management → Corrective Actions. The register shows action type, priority (Critical / High / Medium / Low), assigned team member, due date, and verification status.
Documents
Go to Safety Management → Documents. Published SOPs, safety policies, and training materials are listed here, organized by a Folders tree on the left (Manuals, Policies, SOPs, and any custom folders, each with a document count). You can read and acknowledge documents. Stat tiles show Total Documents, Published, Drafts, and Overdue Reviews.
Risk Assessments (FRAT)
Go to Preflight → Risk Assessments. This console shows every Flight Risk Assessment submitted by pilots, including status (Open / In Progress / Complete) and a risk chip with the numeric score.
Use the Analytics dashboards
Analytics gives you the structured, Part 5–oriented view of organizational safety performance. Select Safety Management → Analytics to expand the group, then choose a view.
You need the View Analytics permission to see these views. Contact your administrator if the Analytics section is not visible.
SMS Performance Assessment
The SMS Performance Assessment report is formatted for Accountable Executive review under 14 CFR Part 5 §5.73. It shows:
KPI Summary — a period-over-period table with change percentages (Total Reports, Reports Closed, Avg Days to Close, New Hazards Identified).
Risk Profile — the distribution across High / Medium / Low.
Corrective Action Performance — CAs completed, CAs overdue, average days to close, and completion rate.
Top Operational Areas — where issues concentrate.
Use the date-range selector and Export PDF to produce records for an audit or board review.
Open Loop Items
Open Loop Items flags audit-readiness gaps: incidents without a linked hazard, hazards without a corrective action, and completed CAs without verification. Drill into each category to see the specific records.
Repeat Event Analysis
Repeat Events opens the Repeat Event Analysis view: recurring safety issues grouped by operational area, location, or aircraft over a 30-, 90-, or 365-day window, with a count and most-recent date per group. Use this to identify systemic patterns.

Troubleshooting
I cannot see Safety Hub in the navigation. You need the Access Safety Hub permission in FSP. Contact your FSP administrator.
I can see Safety Hub but the Analytics section is missing. The View Analytics permission is not on your FSP role. Ask your administrator to add it.
Records from some teams are not visible. Confirm with your administrator that you are designated as the Accountable Executive under Settings → Safety Team — not just a regular team member. Only the AE (and Safety Managers) have the cross-team visibility bypass.
My name does not appear in the Accountable Executive picker. Your FSP user record must be flagged as a Safety Team member before Safety Hub shows you in any picker. Ask your FSP administrator to enable that flag on your profile.
