Safety Hub for Safety Team members

Safety Hub for pilots

Safety Hub gives you a single place to stay on top of your safety tasks. This article is for safety team members who have been granted access to Safety Hub by their organization.

What you can do in Safety Hub

Once you have Safety Hub access, you'll see these areas in the left nav:

  • Dashboard — an overview of your safety information.

  • Safety Reports — file a report about any safety-related event.

  • Hazards — view and track identified hazards.

  • Corrective Actions — follow corrective actions through to completion.

  • Documents — read and acknowledge documents your safety team assigns you.

  • Risk Assessments (FRAT) — complete a Flight Risk Assessment before each flight.

  • Weight and Balance — review your weight-and-balance submissions.

Your organization controls what you can see and do in each area. If something is missing, contact your administrator.

To open a new FRAT:

  1. In the Safety Hub left nav, select Risk Assessments (under the Preflight group).

  2. Select Add Assessment.

  3. Choose the assessment form that matches your flight type (for example, Day VFR or Night IFR).

  4. Select your flight reservation, or choose Manual Entry if no reservation is listed.

  5. Enter your departure airport. This lets Safety Hub generate an AI weather summary for the route.

  6. Answer each question. Safety Hub scores each answer and shows a running total.

  7. Review the Risk Summary (Initial / Mitigation / Final score). If mitigations are available, apply any that reduce your risk.

  8. Select Submit when you are done.

In the Risk Assessments list, each assessment shows a StatusOpen, In Progress, or Complete — and a Risk chip with the risk level and its numeric score, for example Low (0) or High (31).

If your final score is above the threshold your organization has set, your instructor may need to review or authorize the flight before dispatch. When you complete a FRAT from the Pilotbase mobile app during Preflight, that authorization happens there before dispatch.

For a step-by-step walkthrough of the full FRAT form, see the "Complete a FRAT assessment" article.

File a safety report

You can file a safety report from inside Safety Hub at any time. Your report routes automatically to the safety team responsible for your location.

To submit a report:

  1. In the left nav, select Safety Reports (under Safety Management).

  2. Select New Report.

  3. Fill in the reporter section: event date, location, description, and any other required fields.

  4. Review your submission, then select Submit Report.

Your report starts at Submitted and moves to Closed once your safety team has worked and closed it out. (Your organization may customize these status names.)

Your organization may also share a public submission link that lets you file a report without logging in. If you use that link, save the Report ID shown after submission — it is the only way to look up your report's status later.

For the full report-filing walkthrough, see the "File a safety report" article.

Troubleshooting

I don't see Safety Hub in the nav. You need the Access Safety Hub permission. Ask your FSP administrator to grant it.

The FRAT AI weather summary isn't loading. Enter a departure airport first. The Conditions Summary only generates once a departure airport is set.

I submitted a report via the public link but can't find it. Use the Check Report Status link your organization provided, and enter the Report ID you received at submission.
