In addition to the questionnaire, you can also create the consent forms for your next trade fair under Lead Forms. The declarations of consent are always created in combination with a questionnaire and then linked to the appropriate view.

If you have already created your questionnaire, select it now to create your consent forms. Or you can create a new questionnaire.

Select the desired questionnaire by clicking it.

Scroll to the bottom of the page to Consent Confirmation. And select +Add Signature Option.

Enter your desired declaration of consent at Text, as it should be displayed in the app. The key in turn specifies how the declaration of consent is to be exported. Then click on Save. If you want to create additional declarations of consent, select + Add Signature Option.

As with the questionnaire, all changes have to be published after creation to be visible in the app. To do this, scroll back to the top of the page and click on publish.
