How to Change the type of a Form Template

Forms in PracticeSpace can be used for multiple purposes:

  • To be shared with clients to collect information, such as Mental Health and Intake forms

  • To be used to capture practitioner records like Clinical Note

If any of these scenarios seem familiar to you, then this article will be helpful

  • You have a template for a form but cannot share it with a client

  • You have a template for a note but cannot add it to the client

2. Navigate to Papers from the left menu

3. Find the template you're looking for in either Form templates or Notes & other Templates

4. Once you found the form, click on it to edit the Form

Form templates of types "Mental Health", "Intake", and "Consent" can be shared with clients. Form templates of types "Clinical Note" and "Other" can only be filled by a practitioner.

5. In the Form Builder view, check the form type.

6. Click on the dropdown to change the form type

7. Click "Save"

8. Click "Back"

9. This form was originally listed in the "Notes & Other Templates" tab but is now listed in the "My Form Templates" tab.

10. This form can now be shared with a client by going to the forms tab on their profile.

11. Click "Share"

12. This will show a list of forms available to share with the client.
