Label Identification Steps

  1. Step 1:

    • The outbound label has a USPS Ground Advantage label.

    • This label should always be placed on flap #3.

    • The flap number must always be visible.

    • The outbound label for USPS Ground Advantage systems always includes the letter "G" in the top left corner of the label.

    • The label includes the address from which the mail-back system was shipped.

    • The label also includes the customer's or patient's name and destination address, where the mail-back system will deliver.

    • Lastly, on the bottom, the label contains the USPS tracking barcode and number with a unique QR code on the bottom right corner.

  2. Step 2: An aerial image of the outbound label has a USPS Ground Advantage label.

  3. Step 3:

    • The return label has a USPS Ground Advantage label.

    • This label should always be placed on flap #4.

    • Flap #4 of the return label for USPS Ground Advantage systems always includes a biohazard print that indicates a specific UN3291 number that must be visible at all times.

    • The return label for USPS Ground Advantage systems always includes the letter "H" in the top left corner of the label.

    • The return label has a USPS Ground Advantage label. Each label should be unique with its own barcode and tracking number.

    • The label includes the address from which the mail-back system was shipped.

    • The label also includes the customer's or patient's name and destination address, where the mail-back system will deliver.

    • Lastly, on the bottom, the label contains the USPS tracking barcode and number.

  4. Step 4:

    • The manifest form can be found on the side of the box.

    • All customer/patient information can be found there as follows: (from top to bottom)

      • Upper small manifest sticker: with QR code on the far right

      • Manifest:

    • Followed by a plastic pouch with the manifest inside containing the following information:

      • Manifest # and QR code on the far right—

        • Note: The manifest number on the sticker above must match the manifest number inside the plastic pouch.

      • Disposal site name:

      • Facility Address:

      • Permit No.

      • Product SKU

      • Description of contents

      • Generator Name

      • Address

      • Phone Number
