Select the 'Questionnaires' page from the left-hand menu.

Click to highlight the questionnaire you would like to copy.

Once the questionnaire has been highlighted, Select the 'Copy Selected Questionnaire' icon.

Once you've made any changes to the copied questionnaire, select 'Save'.
