Create and Use Forms (Pyn 2)

Forms in Pyn 2 allow you to collect structured input directly from employees and managers—such as nominating onboarding buddies, gathering feedback, or capturing preferences and survey responses

Once built, forms can be embedded into automated flows or messages across your employee journeys.

1. When to Use Forms vs. Polls

  • Forms: Best for multi-question inputs, open-ended feedback, or creating relational updates on employee profiles (such as assigning a Buddy or Mentor).

  • Polls: Best for quick, 1-click sentiment checks sent directly inside a Slack message.

2. How to Create a New Form

  1. Navigate to Forms

    • From the left-hand navigation sidebar, click Flows.

    • Under the Forms sub-navigation, select Forms.

    • In the top-right corner, click the blue Add Form button.

  2. Name Your Form

    • Click into Enter a Form Title at the top left and give your form a recognizable name (e.g., New Starter Feedback, Buddy Selection, or Remote Work Check-in).

    Tip: Naming your form clearly will make it easy to find when embedding it inside flow messages later.

  3. Add Fields to Your Form

    • Click Add Field to open the field picker modal.

    • Choose from two field categories:

      • Common Fields:

        • Checkbox: Captures a boolean (true/false) response—ideal for agreements, confirmations, or opt-outs.

        • Content: Inserts a rich text block (supporting bold, italics, links, and inline images) to provide instructions or context within the form.

        • Likert Scale: Adds a 5-point rating scale (from Very Dissatisfied to Very Satisfied) to gauge sentiment.

        • Text: Provides a multi-line open text box for open-ended written answers.

      • Employee Fields:

        • Buddy: Lets managers select an active employee from your organization's directory to assign as an onboarding buddy.

        • Mentor: Lets managers assign a company mentor to the employee.

  4. Configure Field Settings

    • For each added field, configure the following:

      • Label: The primary question or prompt shown to the user (e.g., "How satisfied are you today?" or "Buddy").

      • Description: Optional descriptive subtext explaining the question or providing guidance.

      • Required: Check this box if the respondent must fill in this field before submitting the form.

      • Confirmation Message: Optional text shown after submission (currently in development).

    • Click Save to confirm each field.

  5. Preview and Publish

    • Click the Preview button in the top right to verify how your questions, scale options, and text fields appear to respondents.

    • When finished, click Publish in the upper-right corner to make the form available across your workspace flows.

3. Embedding a Form into a Flow Message

Once a form is published, you can embed it into any automated Slack, Teams, or email step:

  1. Go to Flows and open the automation where you want the form delivered.

  2. Select the message step (e.g., Send Direct Message or Send Email) to open the editor panel on the right.

  3. Click into the message body where you want the form to appear.

  4. In the formatting toolbar, click the puzzle icon (Widgets) and select Embed Form.

  5. Search for your form name, select it from the list, and click Save.
