Am I Eligible for GLP-1 Treatment?

General eligibility criteria include a BMI of 27 or higher and at least one weight-related health condition (such as prediabetes). A licensed medical provider will review your medical history and make the final eligibility determination.

How to Check Your Eligibility

  1. Log in to your secure Member Portal

  2. Complete the medical intake form accurately

  3. A licensed provider will review and confirm if you qualify

If your BMI is below 27, a provider will still review your case considering any weight-related health conditions. Do not alter your weight temporarily for eligibility purposes — provide accurate health information.

Types of GLP-1 Medications We Offer

  • Branded Options — Ozempic, Wegovy, Zepbound, Saxenda

  • Compounded GLP-1s — Prepared by our U.S. partner pharmacies PerfectRX and ProRX. Compounded drug products are not approved or evaluated for safety, effectiveness, or quality by the FDA. Rx required. Availability of compounded medications is subject to current regulatory status and may vary. Our providers will confirm which options are available to you during your consultation.

Formulation Details

  • Pure Semaglutide vs. Semaglutide with B12 — The formulation depends on the prescription and partner pharmacy. Discuss preferences with your provider.

  • Additives — Some formulations may include Glycine, Vitamin B12, or L-Carnitine. Share prior formulation preferences with your provider during intake.

  • Preservatives — Information about preservatives (e.g., benzyl alcohol) will be on the vial or package insert from the dispensing pharmacy.

Medication Forms

GLP-1 medications are typically supplied in vials, not in one-time-use pen applicators.

Ordering and Shipping

  • Medications ship directly to your door in discreet packaging from our U.S. partner pharmacies

  • A year's supply does not ship all at once — shipping schedules (e.g., quarterly 3-month supplies) ensure freshness and proper storage

  • Tracking information is sent once your order ships

Important Considerations

  • Quality Concerns — If your medication appears cloudy, has sediment, or looks abnormal, do not use it. Contact us immediately with clear photos of the vial(s) and prescription label.

  • HSA/FSA — You can use your HSA or FSA to pay for treatment. We can provide itemized receipts and a Letter of Medical Necessity (LMN) if required for reimbursement.
