How to Run the Service Log Matrix (SLM) Report

Overview

The Service Log Matrix (SLM) Report provides Directors of Rehabilitation (DORs) with a nightly updated report of all patient service logs. This report allows for easy access to service details, reducing wait times and improving readability.

Purpose

The SLM Report enables DORs to quickly access service log data for all or specific patients. The report is updated nightly and is available through the Department Dashboard in RESTORE-Insights.

Key Features

  • Location on platform: Department Dashboard

  • Availability: Updated nightly, reflecting the previous day’s service logs

  • Patient Selection: Choose all patients or search for a specific patient

    • Note: When searching for patients the "All Patients" option will disappear; as soon as you clear the search bar, "All Patients" options will appear

  • File Format: Downloads as a ZIP file, which contains a single PDF with all selected patient records

  • Improved Readability:

    • Clearer formatting for better visibility

    • Payer name displayed prominently

    • Shows when data was last updated

  • Faster Load Time:

    • No more long waits—generates in seconds, compared to the previous 10-minute load times

Important Notes:

  • When searching for a patient, you can use last name, first name, or MRN—but not a combination of these. The search is sensitive, so ensure you enter only one identifier at a time for accurate results.

  • If a report needs to include data from a SOAP note generated on the same day, users must request the report from our team, as it won’t be included in the nightly update.

How to Generate the SLM Report

  1. Navigate to the Department Dashboard in RESTORE-Insights.

  2. Select the Month for which you need the report.

  3. Choose Patients:

    • Select all patients or

    • Use the search feature to find a specific patient.

  4. Click Download—the report will generate in seconds and download as a ZIP file.

  5. Open the ZIP file to access the single PDF containing all patient data.

Best Practices & Potential Challenges

1. Download Behavior

  • The report downloads as a ZIP file instead of a direct PDF. Once unzipped, it contains a single PDF with all patient data.

  • If you navigate elsewhere in RESTORE-Insights while the download is happening, it will still complete.

  • If you change browser tabs during the download, the report will still process normally.

2. Avoiding Download Issues

  • Do not refresh or close the browser tab while the report is downloading—this will cancel the download.

  • If the report doesn’t appear in your Downloads folder, check for a ZIP file instead of a direct PDF.
