📄 How to Download or Print a Submitted Form

Keep a backup, share with your team, or upload to your EHR—right from Aloha.

Once your patients complete an online form, a PDF copy is automatically stored in Aloha—so you always have a backup, even if your EHR isn’t accessible.

🖨️ To download or print a submitted form:

  1. Go to the FORMS tab > click Show Forms

  2. Under the # Submissions column, click the number next to the clipboard icon

  3. Locate the submitted form you’d like to access

    • Click the clipboard icon on the far right of that submission

  4. To download the form as a PDF, click the down arrow (⬇️) at the top right

  5. To print the form, click the printer icon (🖨️) at the top right
