The Patient Profile is broken down into the Contact Card and ten different tabs:

The Contact Card is the information listed at the top of the Patient Profile, and is what follows the patient throughout their entire experience in Clerk - from Check In to Check Out. The Contact Card hosts the following information if added to the Patient Profile:

  • Name

  • Age, gender, and Case Type

  • The name of the Careplan they are on

  • The number of services remaining in the Careplan

  • The Disclaimer Note (an editable Note that is great for logging information you need to know about the patient at every visit, like if the patient is hard of hearing)

  • The Next Appointment date

  • The Last Appointment date

  • The patient's current Balance / Amount Due

  • The patient's Available Credit

Below the Contact Card are the ten tabs that each hold different information specific to that patient:

  • The Details tab is where you will find the patient's demographics, related people, Insurance information, attorney contact information, and medical history.

  • The Appointments tab will show all of the patient's Appointment history and the status of each Appointment. This is also a shortcut to access the Outcome page from any Appointment if you needed to retroactively update anything.

  • The Files tab will allow access to files such as PDFs or pictures. Clerk will allow you to archive files, however, with the simple flip of a toggle, you can pull up any previously archived files. Files that are not archived are also accessible on the Outcome screen.

  • The Chart Notes tab will give you an overview of all the SOAP Notes entered in for a patient by Appointment Date. This tab will also give you the option to download Notes by date range, Appointment Date, or by Note Type for a single day.

  • The Financials tab is where you will find the full transaction history for your patient, including the Superbill, invoices, claims, and subscriptions.

  • The Careplans tab will show current/Active and past/Inactive Careplans that your patient was or is enrolled in. This tab is where you can go in and manually edit a Careplan if you have to.

  • The Alerts tab allows you to create patient-specific notifications that will show up on the Practice Manager. This tab will show all past, present, and future Alerts for your patient.

  • The Contact History tab can show you a log of all outgoing messages made through Clerk.

  • The Pay Integration tab will allow you to keep track of your patient's credit card information and recurring payments if you are integrated with our Payment Management System.

  • The General Notes tab can assist you in keeping track of patient information that is separate from their treatment information. You can edit, archive, and delete Notes as needed. You can see when the Note was created, as well as who created the Note.
