Daily Health Assessment Form: Create on Rhumbix Dashboard

In light of the COVID-19 Pandemic, our team at Rhumbix created two new Field Forms to assess and track your team members's health and wellness while on construction sites. These two new forms are the Daily Health Assessment and Crew Health Assessment Forms. This guide will outline the Daily Health Assessment Form with instructions on how to create and complete one on the Rhumbix Dashboard.

Daily Health Assessment Form

The Daily Health Assessment Form is available to capture data from visitors to the jobsite and onsite office staff such as PMs, PEs, Superintendents, and Accounting. This form can be filled out on the Rhumbix Dashboard and the Rhumbix App.

To create a Daily Health Assessment Form on the Rhumbix Dashboard:

  1. Navigate to "Daily Health Assessment" under Field Forms in the main menu.

  2. Set your project and date filters at the top of the page and then click "Create" on the Action Bar.

  3. The date created, project name, and job number will be pre-filled automatically from pre-existing data. Enter your name, or the name of your employee, if you are filling out the form for them. Following that, proceed to answer the health questions below.

  4. Towards the bottom of the form, you can also add supplemental photos. To do so, click on "Upload Photos" in the blue box to the left.

  5. Add your signature to the form. You can do so by clicking "Add Signature" in the blue box, and then typing your name in the pop-up window. When complete, click "Sign" in the green box.

  6. Review the data entered on the form. When complete, the last step is to click "Save" on the top right.

  7. Your form is now complete! If needed, you can make edits by clicking on "Edit" on the top right of the page. You can also delete the form by clicking on "Delete" in the red box on the top right.

After completing the form, the form will also appear in your list view on the Rhumbix Dashboard with data pulled from the form. On the action bar at the top of the page, you can create a new form, change your grouping, and export this table via CSV or Excel Spreadsheet.
