Crew Health Assessment Form: Create on Rhumbix Dashboard

In light of the COVID-19 Pandemic, our team at Rhumbix created two new Field Forms to assess and track your team members's health and wellness while on construction sites. These two new forms are the Daily Health Assessment and Crew Health Assessment Forms. This guide will outline the Crew Health Assessment Form with instructions on how to create and complete one on the Rhumbix Dashboard.

Crew Health Assessment Form

The Crew Health Assessment Form is available for foremen to capture health assessment data for their crew in the field. This form can also be filled out via the Rhumbix Dashboard or the Rhumbix App.

To create a Crew Health Assessment Form on the Rhumbix Dashboard:

  1. Navigate to "Crew Health Assessment" under Field Forms in the main menu.

  2. Set your date and project filters at the top of the page and then click on "Create" in the action bar.

  3. The date created, project name, and job number will be pre-filled automatically from pre-existing data. In the dropdown menu next to "Health Summary", you can report if your team is "All Healthy" or "Needs Review".

  4. Next, under Labor, click on "Add Row." Here, you can add individual team members and indicate their health conditions in the fields to the right.

    Flu-like symptoms: Mark yes if your teammate has experienced any flu-like symptoms in the past 14 days. Otherwise, select no.
    Near Diagnosed: Mark yes if your teammate has been within 6 feet of anyone diagnosed positive for COVID-19 in the past 14 days. Otherwise, select no.
    Travel: Mark yes if your teammate has traveled to any country with a level-3 warning for COVID-19 in the past 14 days. Otherwise, select no.
    Quarantine or Isolation: Mark yes if your teammate has been placed under an order of isolation or quarantine in the last 14 days. Otherwise, select no.

  5. Towards the bottom of the form, you can also add supplemental photos. To do so, click on "Upload Photos" in the blue box to the left.

  6. Add your signature to the form. You can do so by clicking "Add Signature" in the blue box, and then typing your name in the pop-up window. When complete, click "Sign" in the green box.

  7. Review the data entered on the form. When complete, the last step is to click "Save" on the top right.

  8. Your form is now complete! If needed, you can make edits by clicking on "Edit" on the top right of the page. You can also delete the form by clicking on "Delete" in the red box on the top right.
