Fixed a bug in the Company Wide Annual Schedule Summary report where the month of May was not showing a total

Added the Health Card Number field to the Online Registration Form as an optional field

Made family number searchable in the family list and billing lists.

Fixed a bug where the the Timeclock settings page would need to be refreshed before displaying the Timeclock questions

Added the name of the online registration form to the top of the online registration form PDF
