Each scorecard has header information about the individual being evaluated and details about the event or task being evaluated.
The user can modify the Supervisor (Team Leader) name from the default Team Leader previously assigned to the Employee.
The header will automatically store the scoring date & time as Event date and time and the Evaluator (Monitor) name. However, the user can change the event details such as date or time.
Event type and Event sub type may be selected from the drop-down menus if Event types and sub types have been added to the Scorecard.
A reference such as a CRM ticket number or customer account number may be entered in the Reference field.
The four fields on the right side : Event duration, Event Type, Event Sub Type, Event reference are not mandatory fields by default. But, when configuring the Scorecard, the admin can make mandatory the Event Type and Event reference fields.
If the Scorecard has Custom Fields, the Evaluator will see these extra fields just below the set header information and these fields will be separated off as you can see in the screenshot below. For some Custom Fields, the Evaluator has to select the input from a pick list and for others the Evaluator will be able to enter free text.
