Start in your Custom Questionnaire. Click "+ Add New Question."

This opens the Properties panel on the right. The first box adds the Question text.

The Nickname is for referencing the question later (i.e. conditional questions).

Helper Text adds the a button next to the question with additional info.

Visualization is for conditional questions, these will be addressed later. Click Save.

Once you click Save, the question text will appear. Now click the "+" button to add response fields.

Look back over to the Properties panel and you'll see new options for the question. Choose the type of responses you'd like to use, This example will use Multiple Choice.

Again, these fields can have a Nickname for reference later.

To add response Options, add it in the "Text" field, then the score for choosing that response, then click "+" to add it to the list. Repeat as necessary. Save when done.

To determine how to calculate the total, switch to the Scoring tab.

Choose the type. Click "Save."
