STEPS

  1. Click S2Partner

  2. Click menu open

  3. Click Supplements

  4. Click menu closed

  5. Click Questionnaire Name

  6. Click Question

  7. Click +

  8. Scoring for questionnaires is reliant on scoreable questions
    ​PLEASE NOTE: Only certain field types have scoreable responses

  9. Click Field Type

  10. Select Scoreable Field Type

  11. Enter Nickname
    ​Keep it simple

  12. Options
    ​Create response options for the respondent to choose from. Each response option should have a score or point value

  13. Enter Text

  14. Enter Score

  15. Click +

  16. Repeat
    ​You need at least 2 options to have a score spread

  17. Click Save
    ​When you're ready

  18. Fill in the required fields

  19. Click Save

  20. Click Scoring

  21. Review Minimum & Maximum Values
    ​Maximum Value must be higher than 1 to have a point spread

  22. Select Score Type
    ​This is how the questionnaire will be scored. The following score types are supported:
    ​- Total
    - Percentage
    - Fraction
    - Categories

  23. Review Format & Example

  24. Click Save
