Click S2Partner
Click menu open
Click Supplements
Click menu closed
Click Custom Questionnaires
Click Questionnaire Name
Click +Add New Group
Groups contain sets of questions
Enter Group Title
Click Save
The new group will appear in the questionnaire
Click on the <Group Name> and then + to add a question to the group
