Bring in your proof of US Citizenship, 2 proofs of Oklahoma residency, DOT Medical Card if you have one and are required to maintain it on file, and your driver license.

You can replace your CDL by going to our online CDL Renewal or Replacement portal.

You can also replace at any of our Service Oklahoma locations and at most Licensed Operators.
