Bring in your proof of US Citizenship, 2 proofs of Oklahoma residency, DOT Medical Card (if applicable and are required to maintain it on file) into any Service Oklahoma testing facility so an examiner can view your documents and issue you the proper paperwork to take to the Licensed Operator (tag agent) so they can print you a replacement license.
