Which documents do I need to present to apply for a REAL ID?

Visit our REAL ID Checklist to understand the documents you need. This checklist will have detailed questions to ensure you choose and bring all necessary documentation for yourself.

Here are the key requirements:

  1. One proof of identity showing your date of birth and full legal name, such as a valid U.S. passport, certified U.S. birth certificate, or valid Permanent Resident Card.

  2. Two proofs of Oklahoma residency showing your name and the mailing address that will appear on your REAL ID. Acceptable documents include a mortgage statement, utility or cell phone bill, vehicle registration, or a bank statement.

  3. Your Social Security number must be provided on the REAL ID application.

  4. Name change documentation (if applicable). If your current legal name differs from the name on your proof of identity, you must provide certified documents for each name change, such as marriage certificates, divorce decrees, or court orders. If you have been divorced and remarried, you must present the full history of name changes.

  5. Immigration Documents (if applicable). You will be asked to present your lawful presence documentation for each renewal or replacement service such as:

    • Valid, unexpired I-551 Resident Alien/Permanent Resident Card

    • Valid, unexpired I-766 Employment Authorization Card

    • Valid, unexpired foreign passport with a valid I-94 card issued by the U.S Department of Homeland Security (DHS) or Immigration (USCIS) Stamp

    • Other USCIS documents may be required depending on your status (DS-2019, I-20, etc.)
