Visit our REAL ID Checklist to understand the documents you need. This checklist will have detailed questions to ensure you choose and bring all necessary documentation for yourself.

Here are the high-level requirements:

  1. ONE proof of identity that shows your date of birth and true full name, such as a valid U.S. passport, certified copy of U.S. birth certificate, valid Permanent Resident Card, etc.

  2. If the true full name shown on your document does not match your current true full name, you must bring additional name change documentation for each name change.

  3. TWO proof of Oklahoma residency that include the first and last name and mailing address that will be shown on your REAL ID driver’s license or identification card. Examples include a mortgage bill, home utility or cell phone bill, vehicle registration card, and bank statement.

  4. Provide Social Security number on REAL ID application
