Once you have finished your IDAP requirements, you will follow these steps.

For Step 2:

Click here to access the online Ignition Interlock History Form.

OR

Click here to download the Ignition Interlock History form and email the completed form to the Board of Tests for Alcohol and Drug Influence (bot@bot.ok.gov) for processing.
