You can request your own Motor Vehicle Report (MVR), which includes your driving record for the past three years, through the following methods:
In person at a Service Oklahoma (SOK) location or a licensed operator
By mail
Fees
The standard fee for an MVR is $25.00.
The fee is waived for individuals 65 and older when requesting their own MVR (per 47 OS § 6-117 D).
If you need a certified copy, there is an additional $3.00 certification fee (per 47 OS § 2-110), making the total cost $28.00.

To request an MVR by mail, send the following items to the address below:
A completed MVR request form
A self-addressed stamped envelope (do not use FedEx or UPS)
Payment for the total amount due

Note: Do not send cash or use cash on delivery (C.O.D.) for mail-in requests.
After submitting your MVR request and payment, you can immediately print a PDF of your report.
If you provide an email address, you will receive a link to download your MVR, which will remain accessible for 30 days.
