You can request your own Motor Vehicle Report (MVR), which includes your driving record for the past three years, through the following methods:

  • In person at a Service Oklahoma (SOK) location or a licensed operator

  • By mail

Fees

  • The standard fee for an MVR is $25.00.

  • The fee is waived for individuals 65 and older when requesting their own MVR (per 47 OS § 6-117 D).

  • If you need a certified copy, there is an additional $3.00 certification fee (per 47 OS § 2-110), making the total cost $28.00.

Payment Methods

  • By mail: Cashier's check, money order, or personal check

  • In person: Cash, credit/debit card, cashier’s check, money order, or personal check

Mail-in Requests

To request an MVR by mail, send the following items to the address below:

  • A completed MVR request form

  • A self-addressed stamped envelope (do not use FedEx or UPS)

  • Payment for the total amount due

Mail to:
Service Oklahoma
Attn: Business Support Services
PO Box 11415
Oklahoma City, OK 73136-0415

Note: Do not send cash or use cash on delivery (C.O.D.) for mail-in requests.

After submitting your MVR request and payment, you can immediately print a PDF of your report.

If you provide an email address, you will receive a link to download your MVR, which will remain accessible for 30 days.
