Commercial Driver License Medical Self-Certification Affidavits and Medical Cards can be done online.

If you are the CDL holder, you can update your CDL DOT Medical Card or Self-Certification status using the online form here at https://oklahoma.gov/service/sokonline/online-dot-medical-update.html

If you are a doctor or employer submitting an updated DOT Medical Card or Self-Certification form on behalf of the CDL driver, you can submit the form via email or mail.
