This guide explains how to access and submit an Incident Report on the mobile app using the Incident Management feature (Read more). If you need information on Incident Notes, please refer to the guide here.
To access the Incident menu, click on the burger menu on the mobile app.
The display should be similar to whatโs shown below.

The Incident menu will show you recent Incident notes. If there are no Incident reports listed on this page, it means that none have been submitted or that they are private.
Select the plus ( + ) icon, and you will be prompted to fill in the required information for the report.
Please ensure that the information you submit is correct, as any changes must be made by your Admin.
You may want to ask your Admin for more information on how the Incident's severity is handled in your organisation. Assigning an Incident ticket as Category 1 will automatically notify the responsible team lead.
You can also attach a form relevant to your Incident report. Clicking on this menu will bring you to a list of forms assigned to you.
๐ก Please keep in mind that only forms you have submitted responses for can be attached here.
If you scroll down, you also have the option to upload documents or images.
Once all fields are filled in correctly, you can tap the โSaveโ button at the top of the screen.
You can also add comments about the Incident by clicking on the speech bubble icon. Any comments added by office staff will also be displayed here.
