In this step we are able to verify the hospital affiliation of a provider. You will see the ability to add a hospital affiliation, send a verification request, log a verification, and review past verification requests. This can be found under the Affiliations and Work History Tab.

The hospital affiliations will auto-populate from the application, but if you need to you can also add a hospital affiliation. To add a hospital affiliation, click on “Add Hospital Affiliation”. 

A section will open to add necessary information for a hospital affiliation. Once complete click “Save”. 

If the hospital affiliation that you had added included an email address, then you can send a verification request from Silversheet. To do, click on “Send Request”. 

After clicking it, a section will open up to select the request form you would like to send. You can then select which hospital to send the verification request to. You are also able to preview the request form before sending it. Once you are ready, click “Send”. 

You will now see that there are details found within the Verification Requests Sent section of this step of when a verification request was sent. 

If you would like to manually log a verification of a hospital affiliation, click on “Log Verification”.

After clicking, a new section will open. Here you will need to upload supporting documentation and enter the source of that documentation. The date and verifier will auto populate. 

You’ve completed this, click “Save”. You will now see additional details stating when the hospital affiliation was verified and who verified the affiliation.
