Reject code IND-941-01 occurs when:
The Spouse SSN in the return has been locked because Social Security Administration records indicate the number belongs to a deceased individual.


Why This Happens:

This error occurs when the Spouse’s SSN matches an SSA record flagged as deceased. It could be due to:

  • Incorrect SSA records

  • A typo in the SSN

  • The spouse being deceased but entered incorrectly

How to Fix It:

  1. Check the SSN:

    • Verify the Spouse’s SSN for typos or mistakes.

  2. If the Spouse Is Alive:

    • Contact the SSA to correct their records.

    • Visit an SSA office with proper ID if needed.

    • Wait for SSA confirmation before resubmitting.

  3. If the Spouse Is Deceased:

    • Use the correct filing status (e.g., Married Filing Jointly for the year of death or Qualifying Widow(er) if eligible).

    • Attach Form 1310 if required.

  4. Enter "414HSUB" on the W-2:

    • This is necessary for the return to process correctly.

  5. Contact the IRS if the Issue Persists:

    • Call 1-800-829-1040 for assistance.

  6. Amend the Return if Needed:

    • If incorrect information was submitted, file Form 1040-X to correct it.
