The delivery fee is applies to all delivery orders by default.
There are a few things that can opt folks out:
In setup -> services -> fees you can set a Free Delivery Threshold value. Orders over this amount will not have the delivery fee charged.
Make Customers fee-exempt on their account page > billing
Make Orders fee-exempt on the order detail page in the the info panel
