Proof of Coverage: What Documents Are Accepted?

Proof of coverage documents must show that you had active health insurance coverage during the reimbursement period.

What information must be included?

Documentation should show:

  • Your name

  • Insurance carrier

  • Plan name

  • Coverage effective date

  • Premium amount, when applicable

Commonly accepted documents

  • Insurance card

  • Coverage confirmation letter

  • Insurance invoice

  • Marketplace enrollment confirmation

  • Carrier account summary

  • Medicare documentation (if applicable)

What documents may be rejected?

Documents may not be accepted if they:

  • Do not identify the insured person

  • Do not show active coverage

  • Are incomplete or unreadable

  • Do not support the reimbursement period
