Before finalizing your plan purchase, you have the option to claim reimbursement for dental, vision, or other premiums on the designated page. This is your last opportunity to make such claims. Once your on this page, you will be unable to go back and make changes to your selected plan. Employees are able to claim for dental, vision, family member with different zip/counties, and much more.
Start by Clicking the + Sign:
Locate and click the + sign to begin adding a new claim.
Select Health Insurance Type:
Choose the type of health insurance for your claim from the following options:
Individual
Student
Medicare
Enter Coverage Details:
Select Who is Covered: Indicate the individual or group covered by this claim.
Start Date: Enter the start date of the coverage.
End Date: Enter the end date of the coverage.
Upload Proof of Coverage (POC): If required, upload the document that verifies your coverage.
Save Your Claim:
Click Save Changes to finalize and add your claim. Make sure all details are correct before saving to ensure accurate processing of your claim.
5. After claiming your premiums, you will be routed to the success page and then to your dashboard, where you can view your finalized plan details and reimbursement status. Ensure that all your claims are accurate, added and complete before advancing to the next steps.
