Templates in Tandem help structure your clinical notes by guiding the AI to generate content in a consistent format and style.
Creating templates
You have two ways in which you can create a template:
Option 1: Start from a blank template

For those who prefer full control over the template’s structure, you can start from a blank template and build your template step by step.
To create a new template, go to Settings → Templates and click on + Create new template.
This is ideal if you want to map directly to an existing format in your medical record system system.
Option 2: Duplicate an existing template

If you have a template that’s close to what you need, you can duplicate it to create a copy. This allows you to modify the existing structure and edit specific sections.
To duplicate a template, open the selected template and press the three dot menu, and then select "Duplicate template"
This lets you fully edit a copy of the template without impacting the original template.
Building templates
Start building your template within the template creator by giving it a name and adding relevant entries.
1. Add entries
A template consists of multiple entries, which together form the structure of the template.
Start building the template by Adding an entry.
2. Modify entries:
After you have added an entry, you have a few different ways in which you can modify the entry:
1. Click and drag to re-order entries or nest entries together. |
2. Edit the name for the entry. |
3. Switch between entry data type |
4. Remove entry |
Data types
Text (regular entry)
When using this entry type, when generating the note using the template, Tandem insert relevant information from the consultation in a text format.
Heading
Headings allow you to group other entries together under the heading.
Example:
Templates are automatically saved, so you can always begin to create a template and return to it later. The template won't be visible to the rest of the users at the clinic unless you share the template.
Sharing a template
After you have created and built your template, or made any edits to an existing one, you need to share it to make it accessible for use within your clinic.

Templates can have different statuses depending on whether they've been shared or edited:

✅ To apply new edits, simply share the template again. The status will revert to Shared, and all users will see the updated version.
Troubleshooting

In some cases, when you try to share a template, Tandem may not be able to fully determine what kind of information is expected under certain headings or entries. This typically happens when:

When this occurs, you'll see a prompt asking you to provide a short explanation. Simply write a brief description of what kind of information should be added under that entry - for example:
"Include relevant patient history related to current complaint" "Summarise next steps for treatment or follow-up"
Once you've added this clarification, you'll be able to complete the sharing process.

If you’ve shared a template and notice that some sections are missing or misinterpreted when used in patient encounters, it may be due to how the template was configured by Tandem’s AI. |
If the output doesn’t reflect what you intended - the best approach is to contact Tandem support.
