Overview

Medical History is the feature that extracts information from your uploaded medical documents and generates a medical history for each health topic. The system processes documents you upload (e.g. records, lab results, visit notes), identifies relevant information by condition, and builds a topic-specific medical history you can view on each topic. This gives you a clear, organized view of what your records say about each condition without reading every document yourself.

How It Works

1. Upload documents : You upload medical documents on the Documents page. See the Medical Documents Management guide for how to upload and organize documents.

2. Processing : The system analyzes your documents, extracts medical information, and categorizes it by health topic.

3. Per-topic history : For each topic in your Health Conditions & Topics list, you get a medical history that summarizes what your uploaded records say about that condition—key events, treatments, dates, and other relevant details.

4. Viewing : You view your medical history for a topic from the topic dashboard (section 2. About My Case ) or from the topic's Medical History page.

Medical History is generated from your documents only. It does not pull from external EHRs in real time; it uses what you have uploaded or what has been retrieved into your vault.

Accessing Medical History

1. From the topic dashboard : Open a health topic from Topics. In section 2. About My Case , use the Medical History preview or link to open the full medical history for that topic.

2. From Topics, open a topic, then go to that topic's Medical History page — use the Medical History preview or link in section 2 (About My Case).

3. On the page : You see the topic header and the medical history for that topic—summaries and extracted information from your documents, organized for that condition.

What You See

• Topic-specific summary : Information from your documents that relates to that health topic.

• Key events and dates : Important events, procedures, or changes identified in your records.

• Treatment and care : Relevant treatments, medications, and care mentioned in your documents.

• Source-based : Content is derived from your uploaded (or retrieved) documents; it is not manually written by you or your care team—it is extracted and summarized by the system.

Prerequisites

• Documents uploaded : Medical History is built from your documents. Upload relevant records on the Documents page for best results.

• Topic on Topics page : The topic must be in your Health Conditions & Topics list on the Topics page to see its medical history (see Topic Management).

• Processing time : New or updated documents may take a short time to be processed before they appear in your medical history.

Medical History vs. Topic Notes

• Medical History : Generated by the system from your uploaded documents. You don't create or edit it directly; it updates as your documents are processed. It is organized by topic and appears in About My Case for each topic.

• Topic Notes : Notes you (or the chat) create and edit for a topic. They are your own notes, not extracted from documents. See the Topic Notes guide for creating and editing notes.

Together, Medical History gives you the "what's in my records" view per topic, and Topic Notes give you your own notes and chat-saved content per topic.

Permissions and Access

• Topic on Topics page : The topic must be in your Health Conditions & Topics list on the Topics page to view its medical history.

• View : If you can view the topic and its About My Case section, you can view the medical history for that topic (subject to your role).

• No direct editing : Medical History is generated from documents; you edit it indirectly by uploading new or updated documents. Your own commentary belongs in Topic Notes.

Troubleshooting

"Medical History is empty or missing for my topic"

• Ensure you have uploaded documents that contain information about that condition. Medical History only shows what can be extracted from your documents.

• Confirm the topic is in your Health Conditions & Topics list on the Topics page.

• Allow a short time after upload for processing; new documents may not appear immediately.

"Information I expect isn't in my medical history"

• The system extracts and summarizes from your documents; it may not capture every detail or every document type equally. Some non-standard or heavily formatted files may be harder to process.

• Upload the relevant documents if you haven't already. See the Medical Documents Management guide.

• For your own commentary or chat-saved content, use Topic Notes.

"I don't see the Medical History link"

• Confirm you're on a topic that is in your Health Conditions & Topics list on the Topics page.

• Check that you're in section 2. About My Case on the topic dashboard; Medical History is linked from there.

Limitations

• Medical History is generated from uploaded (or retrieved) documents only; it does not sync in real time with external EHRs.

• Extraction quality depends on document format and content; some documents may contribute little or no text for a given topic.

• The system does not interpret medical images (e.g. X-rays, MRIs) themselves; it can only use text from reports or accompanying documentation.

• You cannot manually edit Medical History; to change what appears, add or update documents. Use Topic Notes for your own notes.
