Overview
Topic notes are your notes for each health topic. They are associated with a specific topic in your Health Conditions & Topics list. You can create and edit topic notes yourself, or the in-app chat can create and edit them for you through tools—for example, when you save a chat message to a topic, it becomes a topic note. Topic notes help you keep observations, questions, and important information organized by condition.
Accessing Topic Notes
1. From the topic dashboard : Open a health topic from Topics. In section 4. Notes & Plans , use the notes area or go to the full topic notes page.
2. From the Tools page : Open Tools from the app and click the Topic Notes (or notes) card. You may be prompted to select a topic.
3. From Topics, open a topic, then use section 4 (Notes & Plans) or the full topic notes page (and the user-notes view when applicable).
Who Can Create and Edit Topic Notes
• You : You can create, edit, and delete your own topic notes when you have the right permissions.
• Chat : The in-app chat can create or edit topic notes on your behalf when you use actions like Save to note on a message, or when the chat uses its tools to add or update notes for a topic. This keeps important answers and summaries in your record without leaving the chat.
Topic notes are always tied to a topic. You need the topic in your Health Conditions & Topics list on the Topics page to add or manage notes for it (see Topic Management).
Topic Notes Layout and Navigation
Reaching Topic Notes
• From topic dashboard : Section 4 (Notes & Plans) shows notes for the topic and an Add Note button when you have permission. Go to the full topic notes page from there.
• From Tools page : Click the notes card and select a topic.
• From topic dashboard or Tools : Use the options above to reach the topic notes page (and user notes when applicable).
Notes Grid
• Notes appear as cards in a responsive grid.
• Each card shows title, content preview, and creation date.
• Add Note : When you have add permission, use the Add Note card to create a new topic note.
• Click a note card to open it for viewing or editing.
Creating a Topic Note
1. Check permission : You need add permission for that patient. If you don't see the add area, you may not have permission.
2. Open the topic : Go to the topic (from Topics or Tools) and open its notes (section 4 or the notes path).
3. Add a note : Click Add Note , enter a title and content, and save. The new note appears in the grid for that topic.
4. Via chat : Use Save to note on a chat message to save it as a topic note; you may be asked to choose a topic. The chat can also create or update topic notes through its tools.
Editing a Topic Note
1. Check permission : You need edit permission for that patient.
2. Open the note : Click the note card to open it for editing.
3. Update content : Change the title or body using the editor. Changes can auto-save when configured that way.
4. Via chat : The chat can edit topic notes through its tools when you ask it to update or add information.
Viewing and Organizing Topic Notes
• By topic : All topic notes for a topic are listed on that topic's notes page. There is no separate "global" notes list; notes are always associated with a topic.
• Rich text : You can use formatting (e.g. bold, lists) where the editor supports it.
• History : For a given note, you may be able to open its history (when available) to see past changes.
Permissions and Access
• Topic on Topics page : The topic must be in your Health Conditions & Topics list on the Topics page to create or manage topic notes for it.
• Add : Requires add permission for the patient; without it, the add-note area is hidden.
• Edit : Requires edit permission for the patient; without it, you can view but not change notes.
• Chat : Saving a message to a note or using chat tools to create/edit notes also requires that you have permission to add or edit notes for that patient and topic.
Troubleshooting
"I don't see the option to add a note"
• Confirm the topic is in your Health Conditions & Topics list on the Topics page and that you have add permission for that patient.
• Refresh the page; if you still don't see it, your role may not include adding notes for this patient.
"My note isn't showing"
• Notes are shown per topic. Ensure you're on the correct topic's notes page and that the topic is in your vault.
• If you saved from chat, confirm you chose the right topic and that the save completed.
"I can't edit a note"
• You need edit permission for the patient. If you only see view, your role may not allow editing.
• Try refreshing; if the edit control still doesn't appear, contact support or your care team.
"Save to note from chat didn't work"
• Confirm you used the Save action on the message (not just Copy). You need permission to add notes for that patient.
• Check the topic you selected and your Topic Notes permissions.
Limitations
• Topic notes are organized by topic; you cannot create free-form notes that are not tied to a health topic in your vault.
• Topic notes require the topic to be in your Health Conditions & Topics list on the Topics page for full access.
• Only topics in your vault can have notes; inactive topics may have limited access.
