Creating and Managing Forms

Create and maintain form templates used to collect traveler, trip, and contact information.

Forms are created and managed from the Forms module. Once a form is created, you can customize its structure, add elements, update content, and prepare it for sharing.


Create a New Form

To create a form:

  1. Navigate to Templates > Forms.

  2. Click + Create Forms Template.

  1. Enter a form name.

  2. Click Create.

The new form opens directly in the Form Builder, where you can begin customizing it.


Open an Existing Form

To edit an existing form:

  1. Navigate to Templates > Forms.

  2. Locate the form you want to update.

  3. Click the form name.

The form opens in the Form Builder, allowing you to modify its structure and settings.


Add Form Details

Every form contains a Form Details section at the top of the builder.

Here you can configure:

  • Form Title

  • Form Description

The title and description are displayed to respondents when they open the form.


Add Form Elements

The Form Elements panel on the left contains all available building blocks for your form.

Elements are grouped into categories:

Travel Information

Trip-related elements such as:

  • Basic Trip Details

  • Trip Notes

Contact Information

Traveler and contact-related elements such as:

  • Basic Details

  • Gender

  • Email

  • Phone

  • Address

  • Birthday

  • Passport

  • Activities & Interests

  • Travel Preferences

  • Secure Travel IDs

Custom Question Elements

Custom questions and response fields such as:

  • Dropdown

  • File Upload

  • Multi Select Options

  • Single Select Option

  • Short Answer

  • Long Answer

  • Rating

  • Date

General Elements

Supporting elements such as:

  • Information

  • E-Signature

  • Terms & Conditions

  • Respondent Details

To add an element, drag it from the left panel and drop it into the form.


Organize the Form Layout

Added elements appear in the form canvas.

You can:

  • Rearrange elements using drag and drop

  • Group related information together

  • Remove elements that are not required

  • Configure field-specific settings

  • Mark fields as required where applicable

The form canvas provides a live view of the structure respondents will complete.


Save Changes

Forms are saved automatically as changes are made.

The save indicator in the top-right corner displays the current save status, helping you confirm that your latest updates have been stored.


Continue Building Your Form

After creating the form structure, you can:

  • Configure individual form elements

  • Preview the form experience

  • Share the form with respondents

  • Review submitted responses
