Step 1: Click "Forms" in the left side navigation panel.
Step 2: Click the "Unum ADA Assist Forms" tab in the top left corner.
Step 3: Click the cloud icon under "Actions" to view the desired form.
Step 4: A new tab will open in your browser displaying the selected form. Download or print a copy of the form by clicking the desired action in the top right corner.
