How to Upload an Employee's ADA Accommodation forms:
Step 1: If an employee has emailed the Admin their ADA Accommodation form without uploading it to the solution, you can easily upload the form to the tool for your review within ADA Assist. Using the left navigation pane, click on the “Records” section. Then, select the Employee/Leave Request you would like to add or view the documents to:
Step 2: From the list, find an employee and click the “Eye” icon to
view the employee’s ADA Accommodation Request:
Step 3: Select Dropdown on Right of "Initial Submission"
Step 4: At the bottom of the Initial Submission section, simply click “Documents” to pull up the documents section:
Step 5: To Add an ADA Accommodation Document, you can select the "+ADD" icon and upload the document to the file:
Step 6: Select the correct "Request" and Select "Type" and upload the correct document:
When the document is successfully added, you will receive a Green Confirmation box that advises the document added successfully.
