If you need to request a change to your work environment due to your medical condition, you can request a change to your work environment as an ADA Accommodation. Once you log into ADA Assist and create your account, you can Request a New Accommodation.

Step 1: From your "Home" screen, navigate to the blue "Request" button on the Request New Accommodation block:

Step 2: Complete the Request section, answering the questions to the best of your ability. You will select, "Change to your work environment" :

Step 3: You will want to complete the questions regarding a request for assistive devices or working conditions (Or both):

Step 3: Complete the full request for leave and then "Submit" your request:

Step 4: You will get a Green Box that appears that will confirm the Submission of your request:

Step 5: If your employer requires that Medical Support Documentation be submitted with your request, you will follow the information in the "Forms" pop-up regarding your submission of the necessary documentation for your ADA Accommodation Request:
