Welcome to the Vault app. Vault is your private space to create, store and when you are ready, submit records of misconduct to your company. These records are stored locally on this device. Nothing is shared with your company until you choose to submit a record.
Tap on Create new report.
Select a Record type. You can click on the ? icon for a description.
Once you've picked your category, tap Create.
Select Add an event and answer the series of questions to the best of your ability. You have the option to skip these and return them later if you prefer
Once you've completed the question flow, tap Review event.
When you feel ready, click Ready to submit.
You will have the option to choose one of our three submission routes.
Next, you will see your assigned case manager and have the option to change them.
Finally, tap Agree & submit.
