Welcome to the Vault app. Vault is your private space to create, store and when you are ready, submit records of misconduct to your company.
These records are stored locally on this device. Nothing is shared with your company until you choose to submit a record.
Select Create new report.
Select a Record type. Select the ? icon for a description.
Once you've chosen your category, select Create.
Select Add an event and answer the series of questions to the best of your ability. You have the option to skip these and return them later if you prefer
Once you've completed the question flow, select Review event.
When you feel ready, select Ready to submit.
You will have the option to submit using one of our three submission routes.
Next, you will see your assigned case manager and have the option to change them.
Finally, select Agree & submit.
