To upload a document follow the steps below.
Typically, you'll see two types of buttons asking you to upload a document: "Choose Files to Upload" or "Select File." Click the button that is presented to you to upload a document.

A pop-up screen will appear. Click the paper icon with a "+" in the middle. This will open up files on your computer. Select the file by double clicking on the file and click the "Upload" button. NOTE: This form must be in a PDF format. If you need help converting your file to a PDF, Adobe has a great, free, and easy tool at this link.
You will know your files uploaded correctly because they will appear in the "Uploaded Files" section or they will appear below the "Select File" button.
